Skilled Nursing Services

Skilled nurses are responsible for delivering and coordinating high-quality patient care in the home, following physician orders and agency policies. Their duties include conducting assessments, planning and implementing care, making referrals, and supervising LPNs and home health aides to ensure competency and safe, effective service delivery. They perform home visits, evaluate patient and family needs, promote safety in the home, and communicate regularly with physicians and the Director of Nursing. Skilled nurses also maintain clear, timely documentation, comply with payer requirements, obtain necessary authorizations, and uphold all agency policies and procedures while ensuring patients receive comprehensive, coordinated care.
